Terbium-161-labeled DOTATOC is a next-generation radiopharmaceutical developed by NRG Pallas for the treatment of somatostatin receptor (SSTR)-positive tumors, including neuroendocrine tumors (NETs) and metastatic castration-resistant prostate cancer (mCRPC). It utilizes the radioisotope Terbium-161, which emits both beta particles and a high yield of Auger and conversion electrons. This combination provides a higher linear energy transfer (LET) and a shorter range compared to Lutetium-177, potentially increasing therapeutic efficacy against small tumor clusters and micrometastases while sparing healthy tissue. The DOTATOC peptide serves as the targeting ligand, binding with high affinity to SSTRs (primarily SSTR2) overexpressed on tumor cells, facilitating internalization and delivery of therapeutic radiation directly to the DNA of the cancer cells.
